It is signed by the President, Mary G Alston, Honorary Secretary, Lillias M Skene, Secretary, GJ Butler, Examiners [one illegible], W Ivon Hayes and Matron M E McDonald. It is undated and unsigned by nurse.

The certificate has black and gold printed text, a gold and black decorative border; it is printed on cream paper. Names and signatures are in black and blue ink.
